Amazon plan of action template: what to include and how to prepare evidence
Use a plan of action when your Amazon notice or Account Health instructions request this kind of explanation. Not every removal, rejection or account issue calls for the same response. This template helps organize what you know and what you have done; it does not replace the instructions for your case or guarantee reinstatement.

Start with the notice, not the template
Record the affected listing or account, the issue Amazon identifies, the requested response, any stated deadline, and where to submit. Check the current notice and Account Health instructions. If Amazon asks for specific documents or a different response process, follow that request rather than sending an unrelated generic plan.
Separate the allegation from your investigation. A notice can identify a problem without establishing why it happened. Review your listing history, relevant records and actual operations. If you believe the action is mistaken, explain the facts and relevant evidence through the route the notice provides; do not invent a fault just to fill a template.
Connect the cause, the correction and prevention
Amazon's official Account Health guidance describes three useful parts: root cause, corrective measures and preventive measures. State only a cause you can substantiate. Then explain the concrete action that addresses the immediate issue and how a process change addresses the underlying cause.
Keep completed and planned work distinct. For completed work, record what changed, when and where you can verify it. For planned work, name the responsible role and target date. Do not describe training, inspections, supplier checks or listing edits as completed unless they actually happened.
Choose evidence for this issue
For a wording issue, relevant material may include the affected text, dated correction records and the product information used to check the new wording. For a quality or condition issue, inspection or handling records may be relevant. Supply-chain documents, authorizations or test reports belong in the response only when requested or genuinely relevant to the issue.
Match each attachment to the point it supports. Use authentic records, check that the product and dates correspond, and follow the notice's document requirements. A long collection of unrelated files does not prove the root cause has been fixed. This preparation checklist does not verify document authenticity.
Avoid responses that leave the question unanswered
Replace ‘we will be more careful’ with a specific check tied to the confirmed cause. Remove copied success stories, unsupported blame, irrelevant business history and promises you cannot demonstrate. If the cause remains unknown, say what is still being checked instead of presenting a guess as a finding.
Before submission, compare your response with every request in the notice. Check the submission route and any deadline again. Amazon decides whether the response is sufficient; neither a template nor a more polished appeal creates a right to reinstatement.
Work through an example
These are illustrative examples, not saved check results or successful appeals.
Illustrative scenario: an unsupported listing claim
We removed the claim and will be more careful.
Review: This does not identify the affected wording, establish why it appeared, or show whether the edit was actually completed.
Next step: If your records support it, identify the affected listing and phrase, describe the confirmed review gap, record the completed edit with its date, and set out a specific pre-publish review step. Attach the relevant correction record only if applicable. No outcome is assumed in this example.
